Where was the warning label?

In case you haven't heard by now, she died of "combined drug intoxication with the sleeping medication chloral hydrate as the major factor."

*Dr. Chip Walls... said chloral hydrate is rarely prescribed and is known to be fatal if combined with certain other drugs - including the sedative Lorazepam, which the autopsy showed she was taking.

"It's very toxic if you mix it with any other central nervous system depressant drugs," Walls said. "You could get profound sedation leading up to coma and respiratory arrest."*


+ The vast majority of us here in the U.S. who buy our prescription drugs from a pharmacy, usually get a bottle which has warning labels on it such as "do not combine with..." The pharmacist, whether he works for a major chain or a small individual store, usually fills out the label.

It seems odd that she didn't get such a warning with hers. Was a pharmacist bypassed in Smith's case, and, if so, one wonders why.
